A 52-year-old woman with an end ileocaecectomy due to previous terminal ileum Crohn’s disease presented with a 3-day history of blood clots in her ileostomy bag. She had no history of diarrhoea or abdominal pain. Her Hb was 54 on her initial presentation. She underwent an ileoscopy with large blood clots found distally but no source of bleeding was detected despite 30 cm of intubation into the small bowel.
